The Accident Incident Reporting System (AIRS) is a confidential tool designed for airlines to analyze human factors in aviation. It helps airlines create an internal system to study human performance related to Airbus aircraft. The main goal of AIRS is to gather feedback from operators about how well these planes perform and to pinpoint significant operational and technical human performance events. By doing this, AIRS aims to enhance our understanding of how these events occur and implement design changes when necessary. It also ensures that lessons learned from these incidents are shared with other operators. Essentially, AIRS answers two key questions: “What happened?” and “Why did it happen?” The analysis relies on a causal factor approach, which uses a structured taxonomy. This taxonomy resembles the SHEL model, taking into account various factors like environmental, informational, personal, and organizational influences on crew actions. AIRS is a vital part of the AIRBUS Flight Operations Monitoring package.
