Most operators, along with many ATS units, aerodromes, and maintenance organizations, have created their own internal safety occurrence reporting systems. These systems typically include a database where safety occurrences are logged. Reports can come from Air Safety Reports (ASRs) or flight data monitoring. It’s important to note that all accidents and certain incidents must be reported to national authorities. This is part of the national accident and serious incident reporting system, often referred to as mandatory occurrence reporting (MOR) systems. To streamline procedures and minimize delays, most organizations utilize ASR forms that align with the national MOR scheme. This consistency helps ensure that if an employer decides to file a MOR, the process is efficient. Overall, the focus is on enhancing safety across the aviation industry.
